This policy explains what information telomerixstemcells.com (the “Site”) collects, how it is used and the choices you have. Health information you provide as a client is governed by our Notice of Privacy Practices.

Information we collect

• Information you send us. When you use a contact or enrollment form you provide the details requested (such as name, email, phone and, for enrollment, date of birth and provider). Form submissions are sent by email to our staff and are not stored on the Site’s database.
• Server logs. Our web server automatically records the IP address, browser type, pages requested and time of each request. Logs are used only for security and troubleshooting and are kept for 12 months.
• Cookies. The Site sets one functional cookie (pll_language) to remember your language choice. It contains no personal data.

What we do not do

The Site does not use analytics, advertising pixels, session-recording tools, embedded chat widgets or any third-party tracking. We do not sell or share your information for marketing.

Third-party services

• Email. Form submissions are delivered through Google Workspace under a Business Associate Agreement.
• Electronic signature. Enrollment forms open on Adobe Sign (documents.adobe.com) in a new tab. Adobe’s privacy policy applies to the information you enter there.
• Client portal. The “Portal” button links to an external patient portal with its own privacy policy.
• Hosting. The Site is hosted on Amazon Web Services in the United States under a Business Associate Agreement.

Security

The Site is served over HTTPS and protected by a content security policy, a web application firewall and access controls. No method of transmission is completely secure; please do not include medical details in the contact form.

Children

The Site is intended for adults. We do not knowingly collect information from children under 13 through the Site.

Your choices and rights

You may request access to, correction of, or deletion of the information you have sent us by writing to admin@telomerixstemcells.com. Requests concerning health records follow the procedure in our Notice of Privacy Practices.
